On visiting to police for medico legal case, after getting
treatment an acknowledgement of receipt is taken for future reference .if
intimation is not given by document and is given by phone then you get a docket
number. After the patient condition is stabilized you can fill a case and
directed to medico legal case. You need to sign on that medico legal form.
Your signature is important for authentication that you
provided that information against doctor. At this point police visits the
hospital and your case will be kept at hospital. They will write down your
statement and ask are you ready to file a case.
The physician who is providing treatment will fill medical
details in that form. Your doctor is asked to give complete report since from
the starting, and then police will assist that report during investigation. You
will get a slip for your case which indicates file number and number of your case.
